    DESCRIPTION

    DESCRIPTION
    The Health Educator II (HE) for The Nleta Project is responsible for delivering comprehensive health education, peer navigation services, and home visiting support to Black Infant Health Program Graduates and their families. This role involves providing culturally sensitive health education, facilitating access to social services, conducting home visits, and assisting families in identifying and accessing the services they need to promote holistic health and well-being.

    EXAMPLES OF DUTIES

    • Develop and deliver culturally relevant health education workshops, seminars, and one-on-one sessions for Black Infant Health Program Graduates and their families.
    • Cover topics such as prenatal care, infant nutrition, breastfeeding support, child development, maternal mental health, and other relevant health and wellness issues.
    • Utilize evidence-based practices and culturally appropriate materials to promote health literacy, self-care practices, and informed decision-making among program participants.
    • Serve as a peer navigator and advocate for families, assisting them in accessing and navigating various social services, including healthcare, housing assistance, food resources, childcare, and mental health support.
    • Conduct assessments to identify families' needs, preferences, and barriers to accessing services, and develop personalized navigation plans to address identified needs.
    • Collaborate with community agencies, service providers, and healthcare professionals to facilitate referrals, coordinate care, and advocate for the needs of program participants.
    • Conduct home visits to Black Infant Health Program Graduates and their families to provide personalized support, education, and resources in a familiar and comfortable environment.
    • Assess the home environment for safety, hygiene, and potential hazards, and provide guidance and resources to promote a healthy and nurturing environment for infants and children.
    • Build trusting relationships with families, actively listening to their concerns, providing emotional support, and empowering them to make informed decisions about their health and well-being.
    • Collaborate with families to conduct comprehensive needs assessments, identifying strengths, challenges, and goals related to maternal and child health, parenting, and family dynamics.
    • Assist families in prioritizing their needs and developing action plans to address immediate concerns and achieve long-term goals.
    • Connect families with community resources, support groups, and educational programs to enhance and strengthen their social support networks and resilience.
    • Demonstrate cultural humility, sensitivity, and awareness in all interactions with program participants, respecting diverse beliefs, values, and traditions.
    • Tailor educational materials and interventions to reflect the cultural and linguistic diversity of program participants, ensuring accessibility and relevance.


    REQUIREMENTS TO FILE

    MINIMUM QUALIFICATIONS
    • High School diploma or equivalent and
    • Two years’ experience in a health education setting. Education may be substituted for experience on a year-for-year basis. *must provide proof if qualifying with degree
    • A valid California motor vehicle operator’s license may be required.
    ADDITIONAL REQUIREMENTS
    • Associate’s degree or coursework in health or human services related field
    • Lived experience in communities served by the Black Infant Health Program or experience navigating similar social, health, or economic challenges is highly valued. This may include personal experience with the healthcare system, social services, or parenting challenges relevant to the role.
