For more than 17 years the Missouri Department of Health and Senior Services in collaboration with the Department of Social Services has provided Money Follows the Person (MFP) services to eligible residents of Skilled Nursing Facilities, helping them to regain their independence by moving from institutional care back to a community setting. - Provide leadership and technical assistance to partnering agencies
- Conduct Level of Care assessments for enrollment into the program and for authorization and maintenance of HCBS services
- Discuss problems and barriers with Show-Me Home contractors to develop resolutions
- Communicate and disseminate Show-Me Home materials
- On a monthly basis, scheduling in-person visits with facilities and other community organizations for the purposes of outreach and education
- Approve person-centered transition plans
- Participate in Continuous Quality Improvement Initiatives
- Plan and organize time effectively and work independently
- Maintain positive working relationships with the public and stakeholders
Minimum Qualifications: - Bachelor’s Degree or higher level degree from an accredited college or university (substitutions may be allowed)
- Lack of post-secondary education will not be used as the sole basis denying consideration to any applicant
- Strong interviewing and communication skills
- HCBS experience is preferred but not mandatory
- Prior experience with customer service and working with the elderly, persons with disabilities or other vulnerable populations is preferred
