What you’ll do: - Serve as an initial point of contact for individuals with intellectual/developmental disabilities, family members, professional partners, and others interested in information and services with the Division;
- Provide on-call phone support for incoming Intake calls;
- Screen referrals received via phone and online for appropriateness and completion;
- Provide information about Division services and the eligibility determination process;
- Interview and assist with completion of application paperwork;
- Prepare and submit application packet materials and record requests;
- Provide Intake Process updates to individual/responsible person(s) as requested;
- Create and maintain electronic case files;
- Gather information utilizing effective interview methods;
- Prepare and send formal letters and correspondence;
- Gather and disseminate electronic and physical mail to appropriate parties;
- Provide information about alternative community resources and services outside of the Division;
- Input all actions and information into multiple sources, systems, and platforms;
- Complete all responsibilities following applicable Federal/State/Department/Division regulations, statutes, waiver assurances, directives, and policies;
- Attend trainings, meetings, and other events virtually and/or in-person, as requested (minimal State-wide travel necessary)
Typical Qualifications: - High school diploma; and
- 0-2 years relevant experience
Preferred Qualifications: - Relevant experience in a human services field;
- Experience working and/or supporting individuals with intellectual or developmental disabilities;
- Proficiency with Microsoft Office and Adobe products, specifically Excel, Word, Outlook, and Acrobat;
- Ability to multitask and prioritize a workload with competing deadlines;
- Strong verbal and written communication skills;
- Capability of maintaining a high level of professionalism regarding consumer confidentiality;
- Strong organization skills and capacity to meet multiple set deadlines;
- Ability to problem solve and take initiative without direct guidance from a supervisor;
- Successful working in a team environment;
- Knowledge of the Division’s eligibility regulations, statutes, waiver assurances, directives, and policies.

Role Description: This position performs an integral function for the Division of Developmental Disabilities’ Intake Unit. This role is responsible for completion of administrative support duties and will assist Intake Specialist’s with duties as assigned as part of the team that determines if an applicant meets eligibility criteria to be served through the Division.
