As a Clinical Caseworker, you will ensure they have the appropriate services and supports to meet all of their needs. Salary: - $42,423.60 - $44,438.16
- Pay commensurate with experience
Examples of Work: - Provide medically related social services for Veterans to attain highest physical, mental and psychosocial well-being through the assessment and care planning process
- Identify and seek ways to support Veterans’ individual needs and preferences, customary routines, concerns and choices
- Make arranges for obtaining adaptive equipment, clothing, and personal items
- Maintain contact with families to report on overall health and planning
- Make referrals and obtain services from outside entities
- Assist with and make referrals for financial and legal matters
- Provide appropriate level of discharge planning
- Provide or arrange for the provision of needed counseling services, participating in behavior management and crisis intervention and planning
- Introduce Veterans and families to the Home, providing information and giving tours
Qualifications: Must demonstrate the following which may be obtained through work experience as a social work professional or completion of a bachelor’s degree in a related program:
- Knowledge of individual, family, and group behavior.
- Knowledge of available community resources.
- Knowledge of medical and treatment records, progress notes, and other related information.
