    Description:

    Jefferson County Human Services has established a new division to coordinate case management, care planning, and make referrals to other resources for persons eligible for any of Colorado’s Medicaid Waivers under Health First Colorado (Colorado's Medicaid program). The new division was created to comply with Federal law that requires case management be provided by an agency that is separate from the agency that provides direct care. The intent is to simplify access for individuals and families and create consistency with services provided across the state. This redesign has created a unique opportunity for someone with strong leadership skills to guide new and seasoned employees in navigating this transition while ensuring the services and needs of our clients and the community are met.

    Jefferson County Human Services is seeking Long Term Care Case Manager Leads responsible for the leading teams handling client-facing work of this division. Long Term Care Case Manager Leads are responsible for providing the Long-Term Care (LTC) supervisors with professional assistance and support. Leads mentor LTC case managers and provide needed trainings. Leads also continue client-facing work such as determining functional eligibility and length of stay for Medicaid Long Term Care Programs, determining appropriate service plan based on needs of individual clients, developing service plans, authorize, set up and maintain community-based services with Medicaid providers, and monitoring services and reassessment needs. Leads also advise case managers on appeal hearing process and participate in hearings as needed.

    We are hiring Long Term Care Case Manger Leads for the following programs:

    IDD (Intellectual and Developmental Disabilities) Program - The teams in this program are responsible for the long-term case management for clients with Intellectual and Developmental Disabilities and their families.

    Non-IDD Program - The teams in this program are responsible for the long-term case management for clients experiencing needs related to physical disability, chronic illnesses, and aging.

    SGF Intake Program - The teams in this program will be focused on the initial enrollment, documentation of case management and monitoring activities, Most in Need assessment, and other issues related to entering clients in the State benefits program. Aspects of this program will also be focused on training and process improvement for the entire agency.

    SCHEDULE:

    The county operates primarily on a 4 day/10-hour work week. (Monday-Thursday); Human Services works a mix of 4 day/10-hour or 5 days a week (Monday-Friday). This position is hybrid, with remote work and in-office work as determined by team and agency needs.

    COMPENSATION:

    Hiring Range: $64,000.00 - $74,000.00 USD Annual

    Compensation will be determined based on education, experience, and skills.

    ESSENTIAL DUTIES:

    Long Term Care Case Manager Lead have the following responsibilities:
    • Maintain ongoing case load.
    • Determine client eligibility
    • Develop appropriate service plans and monitor case load activities to ensure clients are receiving quality care.
    • Develop training programs on regulations and waiver changes for case managers.
    • Maintain and update policies and procedures as needed.
    • Provide outreach and Jefferson County representation at various community groups and agencies, providers and State meetings.
    • Manage decisions and objectives for active individual cases.
    • Coordinate and implement strategies for case managers.
    • Provide consultation and guidance to case managers requiring assistance with case questions, concerns or issues.
    • Assist supervisor with overseeing daily functions and operations within the case management unit.
    • Review transfer packets.
    • Review referrals and manage case assignments.
    • Complete monthly stats and complete internal as well as external state audits.
    • Other duties and responsibilities as assigned.


    QUALIFICATIONS:

    Minimum Qualifications:
    • Bachelor’s Degree in a related field
    • Three (3) years of work-related experience
    • Note an equivalent combination of education and experience is acceptable.
