What You'll Be DoingJoin the Nurse-Family Partnership (NFP) team at Wake County Health and Human Services as a
Nurse Home Visitor (NHV) and help women and families build brighter, healthier futures.
As an NHV, you’ll deliver personalized, in-home support to women during pregnancy and postpartum - empowering them with the care, guidance, and resources they need to thrive. You’ll work closely with a multidisciplinary team, following the evidence-based NFP model and upholding the highest standards of clinical nursing practice.
What You’ll Do:
- Partner with first-time moms to create personalized care plans that support their health, well-being, and parenting success
- Monitor and assess client progress, providing timely support and interventions
- Screen for and provide counseling around prenatal/postpartum depression, making referrals when needed
- Stay up to date on healthcare best practices to deliver safe, high-quality nursing services
- Build meaningful relationships with families and collaborate with healthcare providers and community partners
- Conduct community outreach and help identify potential NFP clients
- Complete all required NFP education and actively develop your professional skills
- Participate in regular case conferences and program evaluations to ensure exceptional care

The Basics (Required Education and Experience) - Graduation from an accredited school of Nursing
- No specific experience required
- RN - Registered Nurse - North Carolina Board of Nursing or Compact state
- BPT - Bloodborne Pathogens Training
- CPR - Cardiac Pulmonary Resuscitation
- AED - Automated External Defibrillator
- Health Insurance Portability and Accountability Act (HIPAA)
- Equivalent education and experience are accepted

Beyond the Basics (Preferred Education and Experience) - Bachelor's degree in Nursing (BSN) strongly preferred
- Maternal and/or pediatric nursing experience
- Home visiting experience
- Public Health experience
