    CalOptima Health is seeking a highly motivated an experienced Supervisor, Therapy Services (PACE) to join our team. The Supervisor Therapy Services for the Program for All-Inclusive Care for the Elderly (PACE) is responsible for supervision and oversight of the CalOptima Health PACE therapy team, as well as the evaluation and assessment of PACE participants to determine mobility level and other physical conditions and functional abilities. The incumbent will lead the PACE Therapy department, including the development of therapy-related policies and procedures for the PACE program. The incumbent will be responsible for providing and recommending therapeutic exercise, gait training, ambulation, etc. The incumbent will be responsible for the planning and development of individual and group exercise programs for maintenance therapy for participants. The incumbent will be responsible for providing a comprehensive physical therapy program for participants of PACE, including development and oversight of functional maintenance programs.

    • Salary Grade: 314 - $99,902 - $159,843 ($48.03 - $76.8476)
    • Work Arrangement: Full Office

    Duties & Responsibilities:
    • 50% - Supervision
      • Participates in a mission-driven culture of high-quality performance, with a member focus on customer service, consistency, dignity and accountability.
      • Assists the team in carrying out department responsibilities and collaborates with others to support short- and long-term goals/priorities for the department.
      • Provides supervision to program aides and other staff who are implementing a physical therapy treatment and/or maintenance program to integrate cost effective measures into rehabilitation practice.
      • Recognizes unit problems and takes responsibility for documentation.
      • Demonstrates active participation in quality improvement (QI) processes.
      • Complies with policies addressing safe working conditions.
      • Monitors unsafe working conditions and recognizes inappropriate and/or ineffective participant care management and resolves issues/problems.
      • Completes written reports.
      • Fulfills calls for department as assigned and adjusts staffing for volume and acuity as necessary.
      • Recruits, hires, develops, trains and evaluates performance of the PACE therapy staff.
      • Leads therapy staff to provide high quality professional services and maintains high standards within the Therapy department.
      • Reviews and oversees documentation by therapy staff.
      • Encourages effective relationships among staff geared to team building and maintenance of a cohesive team.
      • Works with PACE administration and direct care staff to develop and revise PACE policies and procedures related to the Therapy department and the provision of therapy services.
      • Maintains up to date knowledge and expertise regarding industry standards.

    • 45% -
      • Program Support Assessment:
        • Collects patient data and completes required forms with appropriate responses according to department and industry standards.
        • Identifies participant's overt problems/needs and sets priorities.
        • Identifies problems requiring further referral and/or follow-up.
        • Observes and records latest diagnostic results.
        • Performs advanced physical therapy observations using critical thinking skills.
      • Planning Care:
        • Develops a plan of care based on physical therapy or occupational therapy process and which incorporates the plans of other disciplines and continuing care needs.
        • Includes the participant/family in developing or revising the plan of care; makes referrals to interdisciplinary support services.
        • Establishes realistic and measurable goals and sets up treatment plans to administer rehabilitation treatment.
        • Completes initial and periodic assessments (minimally every 6 months) prior to scheduled team meetings.
        • Communicates participant changes to team members.
      • Implementing/Managing Care:
        • Ensures that therapy being provided conforms to accepted industry practice standards and organizational/department standards.
        • Provides correct treatments/procedures and other care as prescribed and according to patient care standards.
        • Demonstrates thorough understanding of age-related characteristics, symptoms and needs of participants served.
        • Explains physical therapy treatment and procedures in appropriate way; provides participant and family education in rehabilitation needs and interventions.
        • Works with other therapists and primary care staff to provide patient care and to implement physician orders as necessary and as appropriate to physical therapy and occupational therapy standards.
        • Documents all physical therapy treatment accurately and within departmental time requirements into patient charts.
        • Understands and demonstrates respect for participant's rights and utilizes established mechanisms for management of ethical issues in participant care.
        • Plans and develops maintenance exercise and therapy programs for participants.
      • Participant/Family Education:
        • Coordinates 24-hour care delivery and provides training to family members, friends CalOptima PACE staff members for those parts of the treatment plan that can be carried out by them.
        • Makes home visits as necessary for home-based interventions and observation.
      • Evaluation:
        • Records participant care delivered as planned and any variation, with appropriate rationale.
        • Makes and records observations, as well as implements rehabilitation measures related to impending or associated problems. Interdisciplinary Team
        • Participates as an active and cooperative member of the Interdisciplinary Team.
        • Attends staff meetings and takes part in participant care planning.

    • 5% - Completes other projects and duties as assigned.

    Minimum Qualifications:
    • Bachelor's degree in physical therapy or occupational therapy PLUS 1 year of experience working with the frail or elderly population required; relevant experience must be within the last 3 years; an equivalent combination of education and experience sufficient to successfully perform the essential duties of the position such as those listed above may also be qualifying.
    • 1 year of supervisory or lead experience overseeing staff that provide therapy services to the elderly required.
    • Have access to means of transportation for work away from the primary office approximately 10% of the time.
    • CPR and First-Aid Certification required or must be obtained within six (6) months of the date of hire.

    Preferred Qualifications:
    • Master's degree in physical therapy or occupational therapy.

    Required Licensure / Certifications:
    • Current Licensed Therapist to practice in the state of California required.

    Knowledge & Abilities:

    • Develop rapport and establish and maintain effective working relationships with CalOptima Health's leadership and staff and external contacts at all levels and with diverse backgrounds.
    • Work independently and exercise sound judgment.
    • Communicate clearly and concisely, both orally and in writing.
    • Work a flexible schedule; available to participate in evening and weekend events.
    • Organize, be analytical, problem-solve and possess project management skills.
    • Work in a fast-paced environment and in an efficient manner.
    • Manage multiple projects and identify opportunities for internal and external collaboration.
    • Motivate and lead multi-program teams and external committees/coalitions.
    • Utilize computer and appropriate software (e.g., Microsoft Office: Word, Outlook, Excel, PowerPoint) and job specific applications/systems to produce correspondence, charts, spreadsheets, and/or other information applicable to the position assignment.

    • Ability to speak (enunciate) clearly in conversation and general communication.
    • Hearing ability for verbal communication/conversation/responses via telephone, telephone systems, and face-to-face interactions.
    • Manual dexterity for typing, writing, standing and reaching, flexibility, body movement for bending, crouching, walking, kneeling and prolonged sitting.
    • Lifting and moving objects, patients and/or equipment more than 50 pounds
