    Minimum Qualifications

    Licensing and Certifications:

    Possession of, or ability to obtain, an appropriate valid California driver's license.

    Education and Experience:

    Two (2) years of paraprofessional and office support work in a Public Health, Health Services, Social Services or Behavioral Health program.

    OR

    An Associates degree with a major in health sciences, social/behavioral sciences, education, nursing, or a closely related field.

    Additional directly related experience and/or education may be substituted.

    Community Health Worker

    DEFINITION
    Under general supervision, performs a variety of paraprofessional and office support duties related to the improvement of family functioning and child and adult services, enters data into a variety of software systems; updates and maintains specialized social records; and performs related duties as assigned.

    DISTINGUISHING CHARACTERISTICS
    This is a single Community Health Worker position. Incumbents assist Public Health staff by relieving them of routine tasks such as performing assessments of clients in the various programs, instructing parents in the development of parenting and household management skills, and/or performing various tasks for the child and adult services programs. Incumbents perform a variety of support work and client support services in Health Programs such as Immunizations, CHDP, MCAH, HIV/Aids Drug Assistance Program, Tobacco Control Education and Prevention, WIC, and Communicable Disease Case Management.

    This class is distinguished from the Office Assistant class series in that Community Health Workers provide both office support and direct client services.

    SUPERVISION RECEIVED AND EXERCISED
    Receives immediate or general supervision from the assigned supervisor and /or manager, depending on areas of work assignments. Exercises no direct supervision over staff.

    EXAMPLES OF ESSENTIAL DUTIES

    • Maintains a routine caseload of families who qualify for Home Visitation Care.
    • Conducts ongoing client home visits, needs assessments, and verifies needs by contacting medical and health services providers.
    • Presents client assessments to professional staff for approval.
    • Completes narrative reports regarding a family dynamic and services provided and/or recommended.
    • Enters data regarding case and client information and contacts into the County EMR and Perpetual Time Study program.
    • Makes client referrals to medical personal or social workers as needed.
    • Provides direct training and counseling to mentally ill and developmentally disabled parents on parenting and household management skills, and teaches families about nutritional meal preparation, budgeting, and household care.
    • Monitors the medical and psychological care of abused children.
    • Observes visits between parents and children for potential referral for CWS/CPS.
    • Provides clerical and administrative support in a variety of health programs.
    • Operates computer systems used in the county.
    • Organizes and enters data into various software, spreadsheets, and word processing programs.
    • Teaches and demonstrates health practices for individual groups.
    • Assists clients in obtaining services and advocates for clients to access resources.
    • May create and develop forms, brochures, newsletters, and informational handouts.
    • Acts as a liaison between clients and professional staff, individuals, and groups in the community serviced.
    • Directly assists families in using and learning about resources of society, particularly medical, legal, and employment services.
    • Maintains records of work performed.
    • Based on the program assignment, may transport children and adults for medical appointments and visits.
    • Posts information to the County EMR and Perpetual Time Study program.
    • Assists with preparation and inventory of educational materials, special preservations.
    • Perform related duties as assigned.
    MINIMUM QUALIFICATIONS
    Knowledge of:
    • Basic rules and regulations regarding Home Visitation Programs and desired outcomes.
    • Department and community resources available to clients.
    • Basic methods and attitudes involved in the in-home care of physically ill, handicapped, or disabled persons, including Children-Family dynamics.
    • Organization, procedures, and policies of the work unit to which assigned.
    • Modern office methods, practices and procedures.
    • File maintenance and information retrieval systems.
    • Computer and software programs used by the department for maintaining and updating records, conduct research, assess information, and/or prepare documentation.
    • Principles and techniques for working with groups and fostering effective team interaction.

    Ability to:
    • Effectively communicate with people from diverse ethnic and cultural backgrounds.
    • Interview clients to obtain a variety of information related to the provision of Home Visiting.
    • Deal tactfully and courteously with the public and other staff when explaining the functions and policies of the work area where assigned.
    • Instruct physically and mentally disabled adults in parenting skills.
    • Deal with physically and sexually abused children.
    • Develop and prepare clear and concise records and reports.
    • Recognize and report specific indications of a need for services.
    • Analyze situations and adopt effective courses of action.
    • Maintain the confidentiality of case records/HIPPA.
    • Regularly work well under pressure, meeting multiple and sometimes conflicting deadlines.
    • Understand and carry out oral and written instructions, work well under pressure and prioritize workload to meet deadlines.
    • Communicate effectively, tactfully and positively both orally and written form.
    • Gather, organize, maintain, and update data, information, records, and reports.
    • Utilize a computer, relevant software applications, and/or other equipment, as assigned.
    • Represent the department and County in a courteous and professional manner in meetings with other departments, the public and private organizations and individuals.
    • Establish and maintain effective working relationships with others and constantly demonstrate cooperative behavior with colleagues, supervisors, customers, and the public.
    • Meet the physical requirements as necessary to safely and effectively perform the assigned duties, which may include, walking on uneven grounds, reaching at/below shoulder level, reaching overhead and lifting

    Sedentary Work: Exerting up to 10 pounds of force occasionally and/or a negligible amount of force frequently or constantly to lift, carry, push, pull or otherwise move objects. Sedentary work involves sitting most of the time. Jobs are sedentary if walking and standing are required only occasionally and all other sedentary criteria are met.

    Light Work: Exerting up to 25 pounds of force occasionally, and/or up to 10 pounds of force frequently, and/or a negligible amount of force constantly to move objects. If the use of arm and/or leg controls requires exertion of forces greater than that for sedentary work and the worker sits most of the time, the job is rated for light work.

    Medium Work: Exerting up to 50 pounds of force occasionally, and/or up to 25 pounds of force frequently, and/or up to 10 pounds of force constantly to move objects.

    Positions in this class may require local and statewide travel as necessary.

    A successful candidate will be required to submit to and pass a background check prior to the appointment. Depending upon the position and/or assigned department, the background check may include but is not limited to civil and criminal history check, reference check, Live Scan, and/or credit check (as allowed under state law).

    Disaster Service Workers: As members of the County of Lake Emergency Services Organization, all Lake County employees are designated as Disaster Service Workers during a proclaimed emergency and may be required to perform certain emergency services at the direction of the department and / or County.

    RETIREMENT - County employees are members of the California Public Employee Retirement system (CalPERS).CalPERS provides a defined benefit pension at retirement for County employees.The pension formulas and employee contribution amounts may vary by bargaining unit, date of hire, prior qualifying service, and classification.     HEALTH AND INSURANCE BENEFITS - Medical, dental, hospital, vision, life and disability coverage is available to employees and their dependents. The County contributes $1,500 of the monthly premiums for the insurance plan options.

    VACATION ACCRUAL may be adjusted for current city or county government employees.

    VACATION - 80 hours per year during the first five years of service, 120 hours per year after five years, and additional accruals based on years of service beyond fifteen years. Pro-rated for part-time employees.

    SICK LEAVE - Accrues at a rate of 96 hours per year. Pro-rated for part-time employees.

    HOLIDAYS- An average of twelve paid 8-hour holidays per year. Pro-rated for part-time employees.

    SALARY - Lake County has an adopted salary schedule. The salary range for most classifications is in five increments with a provision for longevity increases. Appointments are normally made at the first step of the salary range.

    SOCIAL SECURITY - County employees participate in Social Security.

    CALIFORNIA STATE DISABILITY INSURANCE - SDI is available to some County employees.

    DEFERRED COMPENSATION PLANS and CREDIT UNION MEMBERSHIPS are available to County employees.

    LOCATION AND SIZE

    Lake County is in Northern California, north of the San Francisco Bay Area.

    Lake County California covers an area of 1,327 square miles.

    HISTORY

    Clear Lake is said to have been formed more than 2 million years ago. Mt. Konocti formed gradually through a number of volcanic episodes. Pomo Indians settled the area over 10,000 years before Spanish soldiers arrived in 1821. By the mid 1800s, the area was being settled by pioneers and Europeans. After being part of Mendocino, Colusa, and Napa counties, Lake County was formed on May 20, 1861. The county’s name derives from the many lakes in the area, most prominently Clear Lake.     Kelseyville Pear Festival
    Held every year on the last Saturday in September in the quaint town of Kelseyville, the event features pear activities and pear packing competitions, cooking demonstrations and pear tastings, quilt show, arts and crafts, fine art show and sale, children’s activities, mule-drawn wagon rides, food, antique tractor exhibit, live music, a parade, and the “Pear Pavilion.” Lake County is the largest supplier of premium fresh pears in California. Since the first commercial Bartlett orchard was planted in the late 1880s, buyers have chosen Lake County Mountain pears.
