    Correctional Health Services - 20% Recruitment and Retention Bonus The County of Los Angeles is currently offering up to 20% bonus for full-time Clinical Pharmacist positions hired by Correctional Health Services and working on-site in a designated facility to meet critical healthcare needs for the uniquely underserved jail population.

    THE MISSION:
    To advance the health of our patients and our communities by providing extraordinary care.

    DEFINITION:
    Provides specialized clinical pharmacy services in the delivery of pharmacotherapeutics or pharmacokinetics in specialized medical care areas; designs, implements and conducts medication usage evaluations; delivers drug information and drug education services.

    Essential Job Functions

    Participates in the development and continual monitoring of patient drug therapy treatment plans and patient outcomes in clinical specialty programs.

    Performs patient assessments and monitors the effects of drug therapy using clinical symptoms, evaluating laboratory data, and other relevant clinical diagnostic techniques.

    Attends daily rounds and medical conferences with physicians in specialty medical areas to evaluate patient drug therapy regimens.

    Interviews patients to obtain information regarding drug use, drug allergies and sensitivities, andmedication histories; assesses compliance and counsels patients on appropriate drug use.

    Participates in ongoing medication usage evaluation programs by assisting in the establishment of criteria for cost effectiveness/control utilization use, process indicators, and monitoring parameters; conducts evaluations to determine compliance to the established medication use evaluation criteria; summarizes results and submits recommendations to the Pharmacy Chief or the Pharmacy and Therapeutics Committee and assists in establishing drug monographs.

    Participates in the establishment of critical pathways to ensure optimal medication usage.

    Provides comprehensive drug information services for healthcare providers and patients, including developing pharmaceutical newsletters, patient education materials, and assessment and evaluation of formulary requests to the Pharmacy and Therapeutics Committee.

    Designs and conducts in-service and educational programs regarding drug therapy for healthcare providers, pharmacists, and students on a variety of health related services.

    Critically analyzes, evaluates, and prepares recommendations of medical literature in relation to medication therapy.

    Interprets and applies pharmacokinetic drug data in recommendations for patient-specific drug dosage regimen; formulates individualized drug regimens based on patient specific parameters, patient diagnosis, concurrent disease states, pharmacokinetics, pharmacoeconomics, and the patient's condition.

    Advises and assists in implementing cost containment strategies for the usage of pharmaceutical products.

    Reviews journals and periodicals and meets with pharmaceutical product specialists to discuss current market conditions, managed care implications, disease state management, and obtain information to implement cost containment strategies and develop therapeutic outcome studies.

    Participates in performance improvement activities to ensureoptimal pharmaceutical care.

    Dispenses medication as necessary.

    Requirements

    SELECTION REQUIREMENTS:
    OPTION I: One year ofspecialized clinical pharmacy experience*.

    OPTION II: Successful completion of a pharmacy residency program** in clinical pharmacy.
    LICENSE: A current license to practice as a Registered Pharmacist*** issued by the California State Board of Pharmacy.

    A valid California Class C Driver License or the ability to utilize an alternative method of transportation when needed to carry out job-related essential functions.

    PHYSICAL CLASS III - Moderate: Includes standing or walking most of the time, with bending, stooping, squatting, twisting, and reaching; includes working on irregular surfaces, occasionally lifting objects weighing over 25 pounds, and frequent lifting of 10-25 pounds.

    SPECIAL REQUIREMENT INFORMATION:
    *Specialized clinical pharmacy experience in the County of Los Angeles is defined as: an in-depth knowledge of a particular field of work or discipline in such areas as Critical Care Units (MICU, SICU, NICU, Burn ICU), Oncology, Parenteral Nutrition, Ambulatory Care, Drug Utilization Review, Anti-Coagulation, Purchasing Consultant, Drug Information, Pharmacokinetics and Pharmacoeconomics or other specialized pharmacy areas. Applicants MUST describe in detail their specialized clinical pharmacy experience.

    **Applicants MUST attach a legible copy of their certificate confirming successful completion of a one-year pharmacy residency program with their application at the time of filing or within 15 calendar days of filing online. Applications submitted without a certificate will be rejected as incomplete.

    In order to receive credit for any college course work, or any type of college degree, such as a Bachelor's, or Master's degree, or for completion of a residency program, you must include a legible copy of the official diploma, official transcripts or official letter from the accredited institution which shows the area of specialization; or official certificates with your application at the time of filing or within 15 calendar days of filing online.

    ***All required licenses and certifications must be active and unrestricted or your application will not be accepted.

    EXAMINATION CONTENT:
    This examination will consist of an interview covering training, experience, personal fitness and general ability to perform the duties of the position, weighted 100%.

    ELIGIBILITY INFORMATION:
    Applications will be processed on an "as-received" basis and those receiving a passing score will be promulgated to the eligible register accordingly.

    The names of candidates receiving a passing score in the examination will be placed on the eligible register in the order of their score group for a period of twelve (12) months following the date of promulgation.

    No person may compete in this examination more than once every twelve (12) months.

    SPECIAL INFORMATION:
    Appointees may be required to work any shift, including evenings, nights, weekends, and holidays.
