    Position Overview
    This position is in the Chronic Disease and Injury Prevention Program of Austin Public Health in the Disease Prevention and Health Promotion Division and will supervise Austin Public Health’s Healthy Living Program. The Healthy Living Program empowers community members to prevent and manage type 2 diabetes through evidence-based, culturally appropriate community-based diabetes self-management and prevention classes, with a focus on uninsured individuals and communities experiencing health disparities. This position will supervise four Community Health Workers who provide the community-based diabetes education and prevention classes and provide quality assurance including management of program data, ongoing training on diabetes topics, and maintenance of the program’s Centers for Disease Control and Prevention and Association of Diabetes Care and Education Specialists ( ADCES ) certifications. This position will also be a working supervisor position that is actively involved in teaching the nutrition components of the classes in community-based settings and may provide individualized diabetes education for a subset of clients that need additional education and support.

    Grant Funded Position
    • This position is grant-funded, meaning its financial support comes from allocated funds that may be modified, reduced, or canceled at the discretion of the City of Austin or the grant provider.

    Immunization Requirement:
    • Due to high-risk areas, Austin Public Health employees must comply with the department’s immunization policy.


    Travel:
    • If you are selected for this position, and meet the Driver Safety Standards in the City of Austin Driver Safety Program, you may drive when necessary to multiple locations as part of your regular job duties. Otherwise, you are responsible for getting to and from these locations.

    Salary and Benefits:
    • Salary is commensurate with experience and qualifications. This position comes with a comprehensive benefits package consisting of medical and dental coverage, paid sick, vacation, and personal time, and a retirement plan.

    Pay Range
    $30.70 - $38.37

    Hours
    Monday - Friday: 8:00 a.m. to 5:00 p.m.
    • Some evening hours may be required to meet the business needs. A hybrid schedule with some days in-office and some days teleworking is possible for this position.

    Duties, Functions and Responsibilities

    City Government
