    Are you a Bachelor's prepared Registered Nurse who wants to work with first-time parents?

    The Health and Environment Department has a rewarding opportunity to join our Nurse Family Partnership (NFP) home visitation program! We are a collaborative and skilled team that is passionate about building trusting, therapeutic relationships with first-time parents and helping them meet their goals for a healthy family.     Why join the NFP Program? We are specially trained nurses who travel within the county to provide support to first-time parents in the home to help families develop healthy habits, maintain safe living environments and for them to be the best parents they can be. We offer a robust orientation and opportunities for continuing education to promote nurses in providing best practices to the families they serve. Furthermore, nurses in this position have autonomy to maintain an appropriate caseload in a hybrid-remote setting.

    Responsibilities

    Specifically, this position:
    • Establishes therapeutic relationships through home visits, phone calls, texting, and Telehealth with first-time pregnant individuals and maintains this relationship with the family until the child’s second birthday.
    • Educates clients about healthy behaviors for a healthy pregnancy, supporting child health and development and improving self efficacy.
    • Assesses the physical, emotional, social, and environmental needs of families related to health and life-course development; creates plans to meet these needs according to program guidelines and the client’s desires.
    • Works alongside families to identify self-directed goals and evaluates progress toward targeted goals.
    • Gathers information from physicians, hospitals, and community resources as appropriate, working with medical professionals as needed, to provide clients with relevant information concerning individual health plans and to ensure appropriate medical care.
    • Maintains sensitive and confidential information in accordance with HIPAA regulations, and maintains accurate and complete client files and records.
    • Analyzes/accesses information and screenings accurately and takes effective action.
    • Maintains knowledge of trends and developments in the nursing field through continuing education activities.
    • Interacts with a service network to ensure access to affordable and appropriate services in a timely manner.
    • May perform communicable disease case investigations and carry out interventions, participate in communicable disease outbreak response activities during a public health emergency.

    We are looking for a candidate that will:
    • Possess general nursing knowledge, principles, practices, objectives, methods, and techniques in assigned area; be skilled in nursing assessment and nursing processes.
    • Have a working knowledge of community resources.
    • Understand the scope and application of laws and regulations pertaining to public health nursing programs.
    • Communicate clearly and concisely, both verbally and in writing, and prepare and maintain clear and concise charts and records.
    • Be proficient with routine software and business applications, including but not limited to spreadsheets, presentation software, EHR and databases.
    • Understand principles and practices of health education, public health promotion, disease prevention, health equity and inclusion, and public health policy.
    • Exercise initiative and judgment in selecting proper evaluation and treatment protocols.
    • Be familiar with treatment/prevention of communicable diseases and health concerns and with safety principles and infection control, both within the patient care environment and the community.
    • Perform CPR and respond in a medical emergency; effectively respond to crisis situations and people in crisis.
    • Ideally, be bilingual in English and Spanish.
    • Enjoy a collaborative team-based approach as well as thrive in independent work.
    • Be passionate for underserved first-time parents reaching their self-directed goals of having a healthy family and a healthy life.

    Qualifications

    All candidates will be screened for the following required minimum qualifications. Any candidate who does not meet all of the qualifications listed as required will be eliminated from the process.
    • Bachelor's Degree, or equivalent combination of education and/or experience, in Nursing from an accredited college or university required. Related education or experience may be substituted on a year for year basis.
    • Licensed to practice as a registered nurse (RN) in the State of Colorado required.
    • One (1) year experience in providing health services as a Registered Nurse required.
    • Possession of CPR and First Aid certifications by date of hire required.
    • Possession of a valid Driver's License by date of hire required.
    • One year (1) of experience in specific nursing program relevant to the role they will perform preferred.

    Supplemental Information

    Schedule
    The typical schedule for this position is 40 hours per week, Monday through Friday with required weekly supervisor and team meetings. Field work scheduling is managed by the employee. Remote work is allowed as appropriate. This position could report to either our Fort Collins or Loveland office.

    Other Things to Know About Our Department
    This position is limited term. Individuals hired into limited term positions are eligible for County benefits, however, the position is limited in duration dependent upon available funding, business need or signed contract or agreement. Nurse Family partnership is a long-standing grant funded program.

    This position will travel within the county to provide NFP services and may provide other public health nursing duties within the Health Department during a Public Health Emergency.

    The Department of Health and Environment has a vaccine requirement. Successful candidates will be required to comply with the vaccine requirements or request a waiver.
