Answers telephone calls to provide information; documents notes and makes appropriate referrals. Receives, interviews and assesses immediate needs; makes intake determinations and processes intakes; contacts families, schools, and community providers. Contributes to the efficiency and effectiveness of the program's service to its customers by offering suggestions and directing or participating as an active member of a work team. Performs various office and administrative support duties, including the operation of standard office equipment. May drive a motor vehicle to transport youth and families to appointments and other facilities. May perform light custodial duties. PHYSICAL DEMANDS Mobility to work in various settings; drive a motor vehicle to transport youth and their families as needed; stamina to stand and/or walk for an extended period of time; ability to use standard office equipment and read printed materials including a computer screen; the ability to communicate in person and/or over the telephone. ABOUT THE POSITION The Community Navigator performs a variety of technical and professional duties to support the safety, well-being, and success of youth and their families. Performs various assessments, behavioral intervention techniques, referral and service linkage, prevention services and education to youth and their families participating in the Truancy Prevention Outreach Program.
