    Location : Central Office - 123 Robert S Kerr Ave; OKC, OK

    Salary : up to $53,847.00, based on education and experience

    Full Time /Part Time : Full time

    Work Schedule : Monday to Friday

    Primary Hours : 8:00 am to 5:00 pm

    Position Description: The Family Service Specialist is responsible for evaluating needs, assisting clients through problem resolution, making referrals to other service organizations, completing eligibility determinations for various social service and assistance programs, as well as determining eligibility for Ryan White Part B Program and coordinating services. This position engages individuals with HIV in lifelong disease management through medication and care adherence and reduction of barriers to care.

    Position Responsibilities/Essential Functions:
    • Collaborate with surveillance staff to locate, re-engage, and provide linkage to care case management services to people who have been diagnosed with HIV and have fallen out of care.
    • Ensure a smooth transition through each step of the engagement/re-engagement process to encourage continued participation in care and treatment. Help individuals develop a system of communication with providers that promotes self-sufficiency and health literacy. Work closely with case management and clinical staff to ensure initial and follow-up care.
    • Support rapid initiation of ART by reducing barriers to medication access including, but not limited to working with Rapid Start staff to initiate ART for newly diagnosed clients, completing client applications, and/or enrollment in pharmaceutical assistance and other programs.
    • Maintain, enter, and track information, outcomes, and other data to improve client adherence, program services, and reporting.
    • Complete Ryan White eligibility screening and bio-psychosocial assessment to identify and reduce barriers. Collaborate with other internal and external staff and conduct a “warm hand-off” to other services and programs.
    • Maintain basic knowledge of community resources and pay sources available. Coordinate referrals and transitions to appropriate pay sources and/or benefits, including Medicare D prescription drug plans and the federal marketplace. Coordinate activities with appropriate entities, including the program’s contracted pharmacy and insurance premium contractor, HDAP case managers, the state Medicaid agencies (Healthcare Authority and DHS), Oklahoma Insurance Commission, Social Security, Medicare, insurance companies, employers, and ACA navigators.
    • Employ positive strengths-based counseling and motivational interviewing techniques in all interactions with individuals and empower them to become actively involved in their care. Provide psychosocial support, including ongoing support and education for coping with stigma, disclosure to family, friends, and/or partners, etc.
    • Prepare and disseminate written communication to clients, applicants, case managers, and providers, scanning and attaching all pertinent documentation to client record.
    • Review health insurance plans for the coverage the plan provides, e.g. prescription coverage and in-network/out-of-network physician status. Coordinate, track, and resolve insurance coverage and premium payment issues with the program insurance and pharmacy contractor. File with insurance carriers for reinstatement of client’s insurance when policies are cancelled due to a variety of reasons.
    • Review incoming HIV Drug Assistance applications for insurance premium and prescription co-pay assistance submitted by external case managers. Reject back for additional information and/or approve/ deny applications based on program policy.
    • Maintain confidential client records and data.
    • Being present in the office is an essential function of this job

    Minimum Qualifications:

    Education and Experience requirements consist of a master’s degree; or a bachelor’s degree and one (1) year of experience in professional social work; or an equivalent combination of education and experience, substituting one (1) year of experience in professional social work for each year of the bachelor’s degree; or three (3) years of experience as a Family Services Specialist.

    Physical Demands and Work Environment:

    Applicants must be willing and able to perform some job-related travel associated with this position. Work is typically performed in an office and clinic setting with a climate-controlled settings and exposure to moderate noise level. While performing the duties of the job, employee is required to talk, stand, walk, and reach with hands and arms. This position requires long periods of standing or sitting and daily use of computer and phone. Being present at the office is an essential function of the job.
